The SBS therapy is aimed at promotion of an adequate development and growth, prevention of complications and, finally, patient’s refusal from parenteral nutrition and change for complete enteral nutrition. Multiple studies prove efficiency of autogenous surgical reconstruction (ASR) of the small intestine in treatment of short bowel syndrome. They can be divided into two large groups: techniques directed to slowing of intestinal transit by formation of valves, anti-peristaltic segments and others; and techniques for intestinal lengthening by means of reconstruction of the small intestine dilated while intestinal adaptation that allows to use all the intestinal mucosa for absorption and prevents development of intestinal content stagnation. Reduction of patient’s need in parenteral nutrition and, finally, refusal from parenteral nutrition is the main target of small intestine ASR in patients with short bowel syndrome. Choice of technique and patients for surgical treatment, time required for surgical treatment of SBS are very important factors for reaching positive results.</p></abstract><trans-abstract xml:lang="ru"><p>Синдром короткой кишки (СКК) является одной из основных причин развития кишечной недостаточности. Терапия СКК заключается в обеспечении пациенту адекватного развития и роста, предупреждении осложнений и в конечном итоге отказе пациентов от парентерального питания и переход на полное энтеральное питание. Многочисленные исследования доказывают эффективность аутогенной хирургической реконструкции (АХР) тонкой кишки при лечении синдрома короткой кишки. Их можно разделить на две большие группы: методики, направленные на замедление кишечного транзита посредством создания клапанов, развернутых антиперистатических сегментов и др.; и методики удлинения кишечника путем реконструкции дилатированной в процессе кишечной адаптации тонкой кишки, позволяющей эффективно использовать всю слизистую кишечника для абсорбции и предотвращающей развитие застоя кишечного содержимого. Сокращение потребности пациента в парентеральном питании и в конечном итоге отказ от парентерального питания является основной целью применения АХР тонкой кишки у пациентов с СКК.
