The comparison group enclosed 30 practically healthy persons aged 42.3 ± 7.0 years, who had no liver pathology and obesity. The laboratory study included the assessment of functional liver tests, lipid spectrum, proinflammatory cytokine levels and epithelial dysfunction markers.Steatosis of the liver was determined with the method of ultrasonic investigation.</p> <p><bold>Results.</bold> In the examined group with NAFLD, there prevailed women (67 %). The structure of complains and concomitant pathology had no marked gender differences. The USI showed the symptoms of liver steatosis in all the patients. Among men and women with NAFLD, normal transaminase levels, cholestasis syndrome, more expressed in men, similar disorders of blood lipid spectrum were registered. The proinflammatory cytokine concentrations in the blood serum of female patients with steatosis were significantly higher than in the comparison group and among males with NAFLD. The disturbance of the endothelial angiogenic function in the form of increased vasculoendothelial growth factor concentration more pronounced in men than in steatosis women was detected (<italic>р</italic> = 0.031).</p> <p><bold>Conclusions.</bold> Among women with NAFLD, slightly expressed cholestasis syndrome, dyslipidemia, signs of systemic inflammation with increased proinflammatory cytokine concentration and disturbance of angiogenic function of endothelium were registered. In men with liver steatosis, there was identified cholestasis syndrome, dyslipidemia, and angiogenic dysfunction of endothelium, more marked than in women. </p></abstract><trans-abstract xml:lang="ru"><p><bold>Цель.</bold> Изучить клиническую картину, функциональные показатели печени, липидный спектр, уровень провоспалительных цитокинов и маркеров дисфункции эндотелия у пациентов с неалкогольной жировой болезнью печени (НАЖБП) с целью выявления гендерных различий.</p> <p><bold>Материалы и методы.</bold> Обследовано 52 человека с НАЖБП на фоне ожирения, средний возраст 43,0 ± 11,1 года, в том числе 17 мужчин (33 %) и 35 женщин (67 %). В группу сравнения вошли 30 практически здоровых лиц (средний возраст 42,3 ± 7,0 года) без патологии печени и ожирения. Лабораторное обследование включало в себя оценку функциональных печеночных тестов, липидного спектра, уровня провоспалительных цитокинов и маркеров эндотелиальной дисфункции. Стеатоз печени определяли методом ультразвукового исследования.</p> <p><bold>Результаты. </bold>В исследуемой группе с НАЖБП преобладали женщины в 67 %. Структура жалоб и сопутствующая патология не имели выраженных гендерных различий. При ультразвуковом исследовании у всех пациентов имелись признаки стеатоза печени. У мужчин и женщин с НАЖБП уровень трансаминаз был в норме, регистрировался синдром холестаза, более выраженный у мужчин, сходные нарушения липидного спектра крови. Концентрации провоспалительных цитокинов в сыворотке крови пациенток женского пола со стеатозом были значимо выше, чем в группе сравнения и у мужчин с НАЖБП. Обнаружено нарушение ангиогенной функции эндотелия в виде повышения концентрации васкулоэндотелиального фактора роста, более выраженное у мужчин в сравнении с женщинами со стеатозом (<italic>р</italic> = 0,031).</p> <p><bold>Выводы. </bold>У женщин с НАЖБП регистрировался маловыраженный синдром холестаза, дислипидемия, признаки системного воспаления с повышением концентрации провоспалительных цитокинов и нарушение ангиогенной функции эндотелия.
