The sample included 53 placental specimens: group 1 – 21 specimens of normally situated placenta, group 2 – 17 specimens of placenta previa without signs of invasion, group 3 – 15 PAS specimens, comprising 7 cases of placenta accreta, 5 of placenta increta, 3 of placenta percreta (invasion degree 3a). Placental expression was studied using monoclonal antibodies to EPO (EPO antibody Rabbit Polyclonal, suitable for Western blot – WB, immunohistoche­mistry – IHC-P, and ELISA; Human, U.S.A.), the intensity of the immunohistochemical reaction was assessed using a semi-quantitative 3-point scale.</p> <p><bold>Results. </bold>Placental EPO expression in abnormal placentation was statistically significantly increased in the capillary endothelial cells of villi (group 1 – 1 (1–2); group 2 – 2 (2–2,5); group 3 – 2 (2–3); <italic>p</italic> &lt; 0,001) showing an increasing trend in PAS. Specifically, in placental invasion, a marked EPO expression was recorded in syncytiotrophoblast (group 1 – 2 (2–2); group 2 – 2 (2–3); group 3 – 3 (2–3); <italic>p</italic><sub>1-3 </sub>&lt; 0,001; <italic>p</italic> &lt; 0,001) and villous stroma macrophages (group 1 – 2 (1–2); group 2 – 2 (2–2,5); group 3 – 2 (2–3); <italic>p</italic> = 0,002), while reduced (moderate) expression was observed in amniotic epithelium (group 1 – 3 (2–3); group 2 – 2 (2–3); group 3 – 2 (2–3); <italic>p</italic><sub>1-3 </sub>= 0,016; <italic>p</italic> = 0,034).</p> <p><bold>Conclusions. </bold>In abnormal placentation, a statistically significant increase was revealed in the number of capillary endothelial cells of placental villi expressing erythropoietin. In PAS, marked erythropoietin expression was observed in syncytiotrophoblast and villous stromal macrophages, with a concurrent decrease in amniotic epithelium. The inclusion of maternal serum erythropoietin concentration measurement in prenatal screening protocols appears promising for assessing the likelihood of placenta previa and placenta accreta.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Цель. </bold>Изучить плацентарную экспрессию эритропоэтина (ЭПО) при предлежании и врастании плаценты (<italic>placenta accreta spectrum –</italic> PAS).</p> <p><bold>Материалы и методы.</bold> Проведено морфологическое исследование плаценты женщин, родоразрешенных в акушерском стационаре Клиники ФГБОУ ВО ЮУГМУ Минздрава России, г. Челябинск (объем выборки 53 препарата): группа 1 – 21 препарат нормально расположенной плаценты, группа 2 – 17 препаратов предлежащей плаценты без признаков врастания, группа 3 – 15 препаратов PAS (7 случаев <italic>placenta </italic><italic>accreta</italic>, 5 – <italic>placenta </italic><italic>increta</italic>, 3 – <italic>placenta </italic><italic>percreta</italic>, степень инвазии 3а). Для изучения плацентарной экспрессии использовались моноклональные антитела к ЭПО (EPO antibody Rabbit Polyclonal WB, IHC-P, ELISA Human, U.S.A.), интенсивность иммуногистохимической реакции оценивалась с помощью визуальной субъективной 3-балльной шкалы.</p> <p><bold>Результаты. </bold>Показатели экспрессии ЭПО в плаценте при аномальной плацентации были статистически значимо повышены в клетках эндотелия капилляров ворсин (группа 1 – 1 (1–2); группа 2 – 2 (2–2,5); группа 3 – 2 (2–3); <italic>р</italic> &lt; 0,001) с тенденцией к увеличению при PAS. Изолированно при врастании плаценты зарегистрирована выраженная экспрессия ЭПО в синцитиотрофобласте (группа 1 – 2 (2–2); группа 2 – 2 (2–3); группа 3 – 3 (2–3); <italic>р</italic><sub>1–3 </sub>&lt; 0,001; <italic>р</italic> &lt; 0,001) и макрофагах стромы ворсин (группа 1 – 2 (1–2); группа 2 – 2 (2–2,5); группа 3 – 2 (2–3); <italic>р</italic> = 0,002), сниженная (умеренная) – при амниотическом эпителии (группа 1 – 3 (2–3); группа 2 – 2 (2–3); группа 3 – 2 (2–3); <italic>р</italic><sub>1–3 </sub>=<sub> </sub>0,016; <italic>р</italic> = 0,034).</p> <p><bold>Выводы. </bold>При аномальной плацентации выявлено статистически значимое увеличение количества эндотелиоцитов капилляров ворсин плаценты, экспрессирующих эритропоэтин. При PAS зарегистрирована выраженная экспрессия эритропоэтина в синцитиотрофобласте и макрофагах стромы ворсин с одновременным ее снижением в амниотическом эпителии.
