The search was conducted using the following keywords: dorsopathy, inflammation, disc extrusion, disc herniation, discogenic radiculopathy, dorsopathy, disc inflammation.</p> <p>Analysis of the obtained data suggests the formation of a new perspective on the problem of inflammation in the area of discogenic radiculopathy through the segregation of certain findings into isolated “paradoxes” or “vicious circles” of the described problem, thereby expanding the epistemological horizons of the pathogenesis of dorsopathies.</p> <p>Aseptic inflammation in the intervertebral disc area is a multiphase process that includes preclinical biomechanical imbalance, the onset of dorsopathy, and the phase of extrusion formation and resolution. The development of inflammation is associated with the contact spread of immune and neuroinflammatory reactions to neural structures, along with simultaneous activation of reparative mechanisms. The proposed approach makes it possible to substantiate preventive and therapeutic strategies aimed at accelerating the resolution of the pathological process.</p></abstract><trans-abstract xml:lang="ru"><p>Проанализированы и обобщены литературные данные открытых источников (eLibrary, Scopus, Web of Science, PubMed) с неограниченной глубиной поиска. Поиск проводился по следующим ключевым словам: дорсопатия, воспаление, экструзия диска, грыжа диска, дискогенная радикулопатия, dorsopathy, disc inflammation. Изучены патофизиологические аспекты процесса локального асептического воспаления в области дискорадикулярного конфликта с позиции отдельного ПДС.</p> <p>Результатами анализа полученных данных представляется формирование нового взгляда на проблему воспаления в области дискорадикулярного конфликта за счет сегрегации ряда сведений в изолированные «парадоксы» или «замкнутые круги» описанной проблемы, что формирует еще более широкий горизонт гностических аспектов патогенеза дорсопатий.</p> <p>Асептическое воспаление в области межпозвонкового диска представляет собой многофазный процесс, включающий доклинический биомеханический дисбаланс, дебют дорсопатии и фазу формирования и санации экструзии. Развитие воспаления связано с контактным распространением иммунных и нейровоспалительных реакций на невральные структуры при одновременной активации репаративных механизмов.
